Payment and Insurance

Chiropractic health care is covered by many insurance policies
OFFICE VISITS OR HOUSE CALLS BY APPOINTMENT
Elaine J. Wagner, D.C.
6033 N. Sheridan Rd.  CW04S-05
Chicago, Illinois 60660
(773)274-6827
Dr. Wagner sees patients by appointment and reserves your time, making every effort to keep that commitment. She asks that you do the same. If for any reason you need to cancel, please do so at least 24 hours in advance so that another person can be scheduled in your place. Unlike most doctors who charge standard fees and see many patients in an hour, Dr. Wagner sets up an entire hour to work with you. A late cancellation costs her time and money. Therefore a charge of $45 will be made for a late cancellation or a missed appointment. Payment is expected at each office visit unless prior arrangements have been made. Usually a significant improvement in your condition will be noted within six treatments.
Will insurance or Workers’ Comp pay for chiropractic treatment? Chiropractors have insurance equality under Illinois State Law. Payment is expected at the time of treatment unless other arrangements have been made in advance regarding insurance coverage. This office will help you complete any forms necessary for you to be reimbursed by your insurance carrier.

Hello, you came recommended by a friend as a referral for chiropractic work to relieve occasional upper back/neck pain. Please let me know if you accept Blue Cross Blue Shield PPO for payment.

I just got this message – better to call me. The cost per session depends on what we do – hypnosis acupuncture, or both. Usually I suggest three sessions for this, with the first being the longest.
